Remote Patient Monitoring (RPM) gives your practice visibility into your patients' health between office visits. TCT Consultants handles the entire program: device procurement and setup, daily data collection and review, patient communication, and compliant billing under Medicare guidelines.

Enrolled patients receive a cellular-connected biometric device such as a blood pressure cuff, glucometer, pulse oximeter, or weight scale. Readings transmit automatically each day. Our clinical staff monitors for out-of-range values and alerts your team when a patient's numbers warrant attention.

How It Works

End-to-End RPM Management for Your Practice

TCT takes care of everything from device setup through monthly billing. Your practice gets the reimbursement and the clinical insight. We serve practices across Islip, Shirley, Centereach, and throughout Long Island and Suffolk County.

  • Patient Eligibility and Enrollment

    We identify Medicare patients who would benefit from daily biometric monitoring, explain the program to them, and obtain consent before enrolling them and ordering their device.

  • Device Setup and Patient Education

    We guide patients through device setup and teach them how to use it correctly. Initial setup is billed under CPT 99453, and the monthly device supply is billed under CPT 99454.

  • Daily Data Collection and Review

    Devices transmit readings automatically each day. Our team reviews incoming data, flags abnormal values, and documents activity in the patient record to support monthly billing requirements.

  • Clinical Alerts and Provider Communication

    When a patient's biometric readings fall outside safe ranges, we contact your clinical team immediately so timely action can be taken before a condition escalates.

  • Monthly Interactive Communication

    We log at least 20 minutes of interactive communication per patient each month (CPT 99457) to review trends, address concerns, and reinforce treatment adherence.

  • Compliant Billing and Reporting

    All activity is tracked and submitted using the correct CPT codes. We provide monthly reports with device data summaries, alert counts, communication logs, and reimbursement estimates.
